FRONTLINE SERVICE: Issuance of Social Case Study Report/ Referral Letter

Requirements:

For Medical Assistance:

  1. Medical Certificate/ Medical Abstract
  2. Updated Hospital Bill or latest doctor’s prescription with amount indicated and stamped by hospital or pharmacy
  3. Barangay Certificate of Residency
  4. Valid identification Card

Fees/ Charges:       None

Duration:                Seven (7) Working Days

How to Avail of the Service:

  1. Registration of client at the logbook
  1. Social Worker conducts an interview to the applicant to gather relevant information and will advise the applicant on what are the requirements to be submitted
  1. Applicant will submit the requirements to be reviewed by the Social Worker and assess the eligibility of assistance.
  1. Applicant will be given a schedule of home visit for further data gathering and validation
  1. Social Worker will conduct home visit, make collateral interviews and issue claim slip to the applicant
  1. Preparation of Social Case Study Report/ Referral Letter by the Social Worker
  1. Release of SCSR/ Referral Letter to the Applicant

 

FRONTLINE SERVICE: Issuance of Certificate of Indigency

Requirements:

  1. Barangay Certificate of Residency
  2. Valid Identification Card
  3. Certificate of Non-Property (from City Assessor’s Office)
  4. Certificate of Tax Exemption (from BIR)

Fees/ Charges:       None

Duration:                Three (3) Working Days

How to Avail of Service:

  1. Registration of client at the logbook
  1. Social Worker conducts an interview to the applicant to gather relevant information and will advise the applicant on what are the requirements to be submitted
  1. Applicant will submit the requirements to be reviewed by the Social Worker and assess the eligibility of assistance.
  1. Applicant will be given a schedule of home visit for further data gathering and validation
  1. Social Worker will conduct home visit, make collateral interviews and issue claim slip to the applicant
  1. Preparation of Certificate of Indigency by the Social Worker
  1. Release of Certificate of Indigency to the Applicant

 

FRONTLINE SERVICE: Pre-Marriage Counseling (PMC) Service

Requirements:

  1. Application form for  Pre-Marriage Counseling (PMC)

Fees/ Charges:       None

Duration:                Two (2)Hours

How to Avail of Service:

  1. Proceed to City Health Services Office (CHSO) at T. Alonzo St., Baguio City
  1. Fill up the information Sheet and applicants will be advised to attend the required seminar on scheduled date.
  1. Attend the Pre-Marriage Counseling (PMC) Seminar on scheduled date
  1. Issuance of PMC Certificate to applicants for submission to the Local Civil Registry Office

 

FRONTLINE SERVICE: Food Assistance

Fees/ Charges:       None

Duration:                One (1) Hour

How to Avail of Service:

  1. Registration of the Client at the Logbook
  1. Social Worker conducts interview and assessment to the applicant
  1. If qualified, social worker prepares Requisition Issuance Slip (RIS)/Requisition Slip and instruct the Administrative Support Staff for the issuance offood items to be given to the applicant
  1. Provision of food commodities to the applicant

 

FRONTLINE SERVICE: Provision of Assistance to Individual in Crisis Situation (AICS)

Requirements:

For Medical Assistance

  1. Medical Certificate/Clinical Abstract
  2. Barangay Certificate of Residency
  3. Valid Identification Card

For Burial Assistance

  1. Death Certificate
  2. Barangay Certificate of Residency
  3. Valid Identification Card
  4. Letter of authorization (if applicable)

Fees/Charges:           None

Duration:                   Three (3) Days

HOW TO AVAIL OF THE SERVICE:

1. Registration of client at the logbook

2. Social Worker conducts an interview to the applicant to gather relevant information and will advise the applicant on what are the requirements to be submitted

3. Applicant will submit the requirements to be reviewed by the Social Worker and assess the eligibility for assistance.

4. Applicant will be given a schedule of home visit for further data gathering and validation

5. Social Worker will conduct home visit, make collateral interviews and issue claim slip to the applicant

6.Preparation of Social Case Study Report by the Social Worker

7.Social worker submits the Social Case Study Report for approval and processing of assistance.

FRONTLINE SERVICES: Issuance of Senior Citizens I.D.s and Purchase Booklets

QUALIFICATIONS:

1.  Must be 60 years old

          2.  Must be a FILIPINO citizen and a resident of Baguio City

REQUIREMENTS:

          1.  Application Form

          2.  Barangay Residence Certificate

          3.  Proof of Age: Birth certificate, Passport or any valid ID indicating one’s birth date

          4.  2 1x1 ID pictures   

          5.  Proof of citizenship (for naturalized Filipino Citizenship and holder of Dual Citizenship).  To be released by the Department of 

     Foreign Affairs

DURATION: One (1) day

HOW TO AVAIL OF THE SENIOR CITiZEN’S ID

1.  Registration of client at logbook

2.  Issuance of application form by the senior citizens volunteers and information of requirements

3.  Submission of application form and its requirements

4.  Assessment of application by the social worker and the OSCA Head

5.  Preparation of Senior Citizens ID

6.  Endorsement of Senior Citizens ID to the Office of the City Mayor for signature

7.  Release of Senior Citizens ID

 

REPLACEMENT OF LOST SENIOR CITIZEN’S ID

1.  Registration of client at the logbook

2.  Submission of Affidavit of Lost

3.  Preparation of Senior Citizens ID

4.  Endorsement of Senior Citizens ID to the Office of the City Mayor for signature

5.  Release of Senior Citizens ID

 

REQUIREMENT FOR PURCHASE BOOKLETS

1.  Senior Citizen’s ID Card

QUALIFICATIONS:

            1.  Must be A Person with Disability

            2.  Must be a FILIPINO citizen and a resident of Baguio City

REQUIREMENTS:

            1.  Application Form

            2.  Barangay Residence Certificate

            3.  Proof of disability:  Medical certificate stating one’s disability

            4.  2 1x1 ID pictures

DURATION: One (1) day

 

HOW TO AVAIL OF THE PWD ID  ( to be renewed every 3 years)

1.  Registration of client at the logbook

2.  Issuance of application form

3.  Submission of application form and its requirements

4.  Assessment of application by the social worker

5.  Preparation of PWD ID

6.  Endorsement of PWD ID to the Office of the City Mayor for signature

7.  Release of PWD ID

1.  PWD’s ID Card

FRONTLINE SERVICE: Provision of Capital Assistance

(Self-Employment Assistance Program

Requirements:

1.  Barangay Certificate of Residency and Barangay Clearance

2.  Business Permit (if applicable)

3.  Picture of Products/Business

4.  Community Tax Certificate (CEDULA)

5.  1 pc 2x2 ID picture

6.  Valid Identification Card

 

Fees/Charges:       None

Duration:                 15 working days

 

HOW TO AVAIL THE SERVICES:

  1. Registration at the logbook
  1. Social Worker conducts an interview and initial assessment to the applicant to gather relevant information and will orient the applicant on the requirements to be submitted.
  1. Applicant will be given a schedule of home visit for validation and further data gathering.
  1. Social Worker shall conduct home visitation and collateral interviews. 
  1. Applicant will submit the requirements to be reviewed by the social worker.
  1. Social Worker will assist the aplicant in the preparation of Feasibility Study/ Project Proposal.
  1. Social Worker will give the Contract to the applicant for signature and notarization.
  1. Applicant will submit the notarized Contract to the social worker.
  1. Preparation of voucher by the Social Worker.
  1.  Social Worker submits the documents for approval and processing of assistance 

 

 

FRONTLINE SERVICE:  ISSUANCE OF SOLO PARENT IDENTIFICATION CARD

Requirements:

  1. Barangay Residency Certificate
  2. Affidavit of two disinterested persons (for unwed, abandoned, separated, widow/widower for more than a year)
  3. Income Tax Return/pay slip (for working applicants)
  4. Photocopy of Birth Certificates for minor children (bring original Birth Certificates)
  5. Medical Certificate for applicants whose souse are disabled (original)
  6. Certificate of Legal Separation (original)
  7. Certificate of Finality of Marriage (original)
  8. Certificate from Jail for applicants whose spouse is in jail for one year
  9. Death Certificate (bring original)

Duration:          Thirty Days (30)
